United Way of Northeast Florida - provides COA the opportunity to provide needed services
and resources to build a stronger community for the elderly

Nassau County Volunteer Center - partnerships with COA to provide volunteers who serve our
fellow citizens and elderly in our community.

Legal Aid of Jacksonville - COA provides space to Legal Aid for those seeking appointments
for legal help.

Sheriffs Department - COA responds to emergency referrals for homeless, isolated or abused
elderly.

Community Action Agency - COA works closely to avoid duplication of service by monitoring
disbursement of funds for low income elderly.

Agency for Health Care Administration - collaborate in the process of identifying and assisting
clients eligible for Medicaid Waiver.

Barnabas Center -  a crisis center that coordinates with COA to provide food and financial
assistance for elderly; expanding the effectiveness of both agencies.

Alzheimer's Association - co-sponsors Memory Walks, provides space and staff support for
caregivers.

Community Hospice of  Northeast Florida - provides staff support, training and education for
terminally ill elderly.

Sutton Place Behavioral Health - COA identifies and refers elderly in need of counseling
services and/or vi
ctims of elder abuse.

Department of Emergency Services - provides transportation to elderly and handicapped during
evacuations and disasters.

Adult Protective Services - provides complete assessment within 24 hours and COA service
delivery is within 72 hours.

Northeast Florida Community Action Agency - promotes public awareness and self sufficiency
through educational and housing programs.  Special Senior projects; Thanksgiving baskets and
Senior Citizen Christmas party & food basket giveaway.

Fire Department - provides education and installation of smoke alarms in elderly homes.

COA cooperates and partners with: Local Church's, Work Source, Voter Registration, Shine Program,
AARP, Local Businesses and Friends of Fernandina Beach in assisting elderly people on a variety of
issues.
